Everything you need before your first visit
Come prepared and your appointment time goes to what matters: your history, your evaluation, and your plan.
Before your visit
Your intake forms should be completed and submitted at least 24 hours before your appointment. If that's not possible, let us know.
Please bring or send ahead of time:
- Prior medical records and imaging
- Recent lab results
- A current list of medications and supplements
- A pair of shorts for the physical evaluation
Your forms
We strongly prefer forms completed online. It allows us to review your history before your appointment and means you arrive ready to get to work.
Call the office at (208) 946-5888 and we'll send your forms over directly.
Insurance and payment
We don't bill insurance directly. We provide everything needed for self-submission to your insurance company, including a superbill. HSA and FSA funds are accepted.
- Medicare: billed directly from our office.
- Lab work: available through a physician co-op with significantly reduced pricing. We'll discuss options before ordering anything.
- Initial evaluation: $225 to $495 depending on complexity, discussed before scheduling.
Cancellation policy
We ask for 24 hours' notice for cancellations. Late cancellations may be subject to a fee. To cancel or reschedule, call (208) 946-5888.

Coming from out of town
Supplements
Our office carries supplements and can arrange access to a patient ordering portal for direct shipment. Ask our team for details.

The groundwork happens before you sit down.
Forms in early, records and labs sent ahead, a list of what you are taking. That preparation is what turns a first appointment into a real conversation instead of a paperwork exercise.
